This multidisciplinary clinic overseen by Lawrence Zeidman, MD, FAAN, offers neurology,PT/OT, and interventional pain management services in one convenient setting. It’sdesigned to work up and medically treat undiagnosed peripheral neuropathy, nervepain from various causes, carpal tunnel syndrome and other entrapment neuropathies,and nerve injuries. Full evaluation will be performed, testing ordered, andtreatment options discussed at the time of the visit.

Dr. Lawrence Zeidman

Dr. Lawrence Zeidman

Lawrence Zeidman, MD, FAAN is board-certified in neurology and clinical neurophysiology and is Associate Professor of Neurology and Rehabilitation,Director of the fully accredited UIC EMG laboratory and the UIC clinical neurophysiology fellowship program. Hecompleted his medical school and neurology training at Northwestern University in 2008, and his fellowship training inclinical neurophysiology at Rush University Medical Center in 2009. He has been on faculty at UIC since 2010. His mainresearch interests are in improving diagnostic techniques in entrapment neuropathies, as well as in diagnosis of small fiberneuropathy.

Khalid Malik, MD

Dr. Khalid Malik

Khalid Malik, MD is a diplomat of the American Board ofAnesthesiology, Critical Care and Pain Medicine. Hecompleted his residency in anesthesiology and fellowshipsin critical care and pain medicine at NorthwesternUniversity, Chicago. He is division chief of PainMedicine and an Associate Professor at the Universityof Illinois, Chicago. He received his medical degreefrom Dow Medical University, Karachi, Pakistan in 1985and also earned the fellowship of the Royal College ofSurgeons (Edinburgh) in 1991. His primary area ofresearch is in spinal pain and related disorders and theuse of minimally invasive techniques to treat these pain syndromes.

Gina Votta Velis, MD, PhD

Dr. Gina Votta Velis

Gina Votta Velis MD, PhD, is a diplomat of the AmericanBoard of Anesthesiology and Pain Medicine. She isan Associate Professor at the University of Illinois atChicago and the Director of the Pain Medicine and theRegional Anesthesia and Acute Pain Medicine FellowshipPrograms. She has received her Masters in Immunologyfrom Claude Bernard University in Lyon Francein 1981, her Medical Degree in 1988 and her PhD in1991, both from the School of Medicine and HealthSciences, University of Athens, Athens, Greece. Shereceived her residency training in Anesthesiology andher fellowship training in Pain Medicine at the Universityof Illinois at Chicago. She has been practicing painmedicine for 15 years.

Amanda Butler, DPT

Amanda Butler

Amanda Butler received her Doctorate in PhysicalTherapy from Utica College and specializes in outpatientorthopedic conditions. She has experience intreating diagnoses including, but not limited to, radiculopathy,balance disorders, and chronic painconditions, and develops a plan of care in order tominimize pain, maximize strength and achieve anoptimal level of function.

Jeannine Lau, OT

Jeannine Lau

Jeannine Lau is a Board-Certified Hand Therapist,specializing in the treatment of injuries and conditionsof the hand and upper extremity. She completed heroccupational therapy training at the University ofIllinois at Chicago, College of Applied Health Sciences.
